Informed Consent During COVID-19

At Katherine Drew Salon, the health and safety of our guests is our highest priority during the COVID-19 pandemic.

COVID-19 is an infectious virus that currently has no direct treatment, and for which no current vaccine exists. While we have taken reasonable steps to limit the potential for transmission of COVID-19 in our salon, you agree and understand transmission of COVID-19 is still possible.

By necessity, hair related beauty services require that our staff and hair stylists be within 6 feet of you and will touch you. You understand that person-to-person contact may increase the chance of COVID-19 transmission. It may be necessary that you quarantine and/or take other steps in the event it is determined that you have been exposed to COVID-19. You fully understand that

recommendations and guidelines regarding COVID-19 are subject to modification.

You understand if you are presenting any of the following symptoms of COVID-19, a temperature of 100.4 or above, shortness of breath, loss of sense of taste or smell, dry cough or a sore throat, if you have been exposed to anyone with these symptoms in the past 14 days or live with anyone who is sick or quarantined, you would need to reschedule your visit to the salon to a later date, 14 days after, symptoms or exposure has passed.

You understand to prevent the spread of contagious viruses and to help protect each other, you will follow

the safety guidelines Katherine Drew Salon Salon has in place, including wearing a mask and having your temperature taken and logged upon arrival to and departure from the salon.

You verify you have not traveled domestically or internationally in the last 14 days by commercial airline, bus or train.

I confirm the personal health information stated above to be accurate. I also agree to indemnify Katherine Drew Salon and it's employees harmless from and against any and all claims, causes of action and damages and costs, including court costs and attorney fees, arising out of any infection and/or illness caused by potential exposure to Covid-19 during your hair service at Katherine Drew Salon.
